Or enter by mail by hand-printing your name, address, age, phone number and email address (if any) on a 3x5 piece of paper, and mailing your completed entry in an envelope to: SPIKES’ Salsa Pantry Promotion Sweepstakes Entries, P.O. Box 578, Penfield, NY 14526. No mechanical reproductions. Each entry must be mailed separately. Limit one (1) online entry per person and per email address per day; Additional online entry attempts on the same day will be rejected, and repeated attempts to submit excess online entries may result in disqualification. Online entries must be received by 11:59 p.m. on 4/ 5 /11; mail-in entries must be postmarked by 4/5/11 and received by 4/19/11. Winner Selection. Prize winner will be selected at random, on or about 5/3/11, from among all eligible entries received. Validations: Entrants agree to be bound by these Rules. Judges decisions are final. Potential winner will be notified by mail and may be required to sign and return an Affidavit of Eligibility & Prize/Publicity Release. Failure to comply may result in disqualification. No responsibility for lost, late, stolen or misdirected mail or entries, or any failure or unavailability of electronic communications or Internet. If integrity of online portion of Sweepstakes is compromised in any manner, Sponsor may modify Sweepstakes and randomly select winner from valid entries unaffected by the problem. Prize/Odds: One (1) Grand Prize: Winner’s choice of one (1) of the following three items, each with an approximate retail value of $500: (a) The Heart Healthy & Wholesome Package (a collection of wholesome products from Sponsor) , (b) The Dorm Room Dream Package (a collection of snack products from Sponsor), or (c) The Fully-Stocked Pantry Package (a gift card for $500 redeemable at a grocery store selected by Sponsor in its discretion in winner’s area). If selected, The Heart Healthy & Wholesome Package, or The Dorm Room Dream Package, will be mailed to winner at the address indicated on the entry form. The Fully-Stocked Pantry Package Gift Card is subject to all terms and conditions on the gift card and cannot be extended or replaced if destroyed, lost, stolen, or unredeemed; use of the gift card for purchase of certain products may be limited if required by law. Gift Card Prize must be used within six months after issue date. Winner is responsible for all other expenses associated with his/her use of the prize not specified here. Odds depend on number of entries. Sweepstakes is open to over 100 million Internet users in the U.S. No substitution or transfer of prizes. If any prize becomes unavailable, Sponsor may award substitute of equal or greater value. Taxes are winner’s responsibility. Allow 4-6 weeks after validation for arrangement for receipt of prize.
